Los trastornos GRIN (acrónimo de trastornos relacionados con el receptor ionotrópico de glutamato, NMDA) son un grupo de trastornos del neurodesarrollo causados por mutaciones en genes que codifican subunidades del receptor N-metil-D-aspartato (NMDA) , lo que provoca una disfunción en la señalización del glutamato . Los trastornos GRIN se caracterizan universalmente por diversos grados de retraso del desarrollo y discapacidad intelectual, así como por crisis epilépticas . Otras manifestaciones clínicas varían según el gen afectado y pueden incluir hipotonía muscular , espasticidad y trastornos del movimiento . Los trastornos GRIN se diagnostican mediante pruebas genéticas y se tratan sintomáticamente, ya que actualmente no existe cura para esta enfermedad.
Presentación
Las características clínicas de los trastornos relacionados con GRIN suelen incluir discapacidad intelectual y epilepsia, junto con otras características neurológicas y/o neuropsiquiátricas, como trastornos del movimiento , trastorno del espectro autista y problemas con el tono muscular . [ 1 ] La tabla a continuación presenta algunos síntomas que pueden estar presentes en trastornos asociados con genes específicos, pero la frecuencia y la gravedad de estos síntomas son variables. Hasta el momento, solo se han reportado unos pocos casos de trastorno relacionado con GRIN2D; por lo tanto, las características enumeradas pueden no reflejar con precisión el espectro de hallazgos en este trastorno.
Pathophysiology
NMDA receptors are a class of ionotropic glutamate receptors. The receptors are clustered at postsynaptic membranes of neurons and are composed of four subunits, which form a pore that allows for the passage of cations, including Ca2+, Na+, and K+. Normally, the pore of the receptor is blocked by Mg2+ in a voltage-dependent manner. Activation of the receptor requires binding of its main agonist, glutamate, as well as its co-agonist, glycine. Upon binding of glutamate and glycine and depolarization of the postsynaptic membrane mediated by another type of ionotropic receptor, AMPA receptor, Mg2+-caused block is relieved. This allows for the passage of Ca2+ and Na+ inside and K+ outside the cell, which leads to further postsynaptic membrane depolarization and activation of multiple downstream signaling pathways. Normal function of NMDA receptors is crucial for neuronal development, synaptic plasticity, learning, and memory.
NMDA receptor is composed of four subunits assembled from seven possible ones (GluN1, GluN2 A to D, and GluN3A and GluN3B).[7] A conventional NMDA receptor includes two GluN1 and two GluN2 subunits. The role of another type of subunits, GluN3, is not yet clear, although both its subtypes (GluN3A and GluN3B) were shown to resemble GluN1 subunits structurally and to assemble with the latter forming a functional receptor.[8] Subunits of NMDA receptor are encoded by GRIN genes:
- GRIN1 codes for GluN1;
- GRIN2A, GRIN2B, GRIN2C, and GRIN2D code for GluN2 A to D;
- GRIN3A and GRIN3B code for GluN3A and GluN3B.
Las variantes de sentido erróneo y sin sentido de estos genes pueden afectar la función del receptor NMDA y potencialmente provocar una serie de secuelas neurológicas y neuropsiquiátricas.
